Between clinic appointments, readings and observations can become scattered across family members. This service brings them into one picture: what to track, how to record it and which questions to raise with the clinician. The aim is continuity and understanding—not replacing the doctor or making treatment decisions from a single reading.

01Does a home visit replace a doctor’s appointment?
No. Home follow-up supports the medical plan and organises information between appointments; clinician reviews remain part of care.
02Do we need nursing throughout the day?
Not necessarily. Scheduled visits may suit needs at specific times. If clinical tasks require a longer presence, the coordinator can discuss extended nursing.
