General educational content pending clinical review before launch. Follow the treating team’s instructions for your individual situation.
Keep the essentials together
Keep the current medication list, clinician’s instructions and upcoming appointments together. Do not upload these to the public form; ask the coordinator how to share them appropriately.
Choose a point of contact
Choose a family contact to coordinate the visit and gather questions, while keeping the patient involved in decisions wherever possible. Write down the two or three questions that matter most.
Prepare a suitable space
Choose a comfortable space with privacy and room for the team to work. Tell the coordinator about stairs, access difficulties or particular needs. Check what is required before buying equipment.
Leave with a clear next step
Before the visit ends, confirm the instructions, follow-up arrangements and who to contact with questions. Ask for an explanation again if anything is unclear.
